Industrial Action Hardship Fund Form
This is the Form to apply for the Hardship Fund. Once Completed it should be sent along with 2 payslips, one before the strike and one with the strike deduction, to the District Treasurer at 18 Ferndale Road, West Knighton, Leicester LE2 6GN.
